We want to do our best to make our clients feel comfortable, safe and satisfied, and even though we are an essential business to our clients, we will remain closed until the state of emergency is lifted and we can continue services as usual. Prior to your appointment, you may download the appropriate intake forms based on your appointment and email the completed document to email@example.com. If you are unable to complete prior, we ask you to arrive 15 minutes before your scheduled time to complete the necessary paperwork.
To provide our clients with the best care and most appropriate treatment plan, we require all of our clients to complete a new health history form on an annual basis.
